Mr. D is a 59-year-old male, currently employed as a long-haul truck driver
  • Former smoker, 35 pack-years, quit 4 years ago
  • Diagnosed with COPD 6 years ago
  • Currently treated with single-inhaler triple therapy at an appropriate dose
  • Reports good adherence and correct inhaler technique
Medical history
  • Hypertension, well-controlled
  • Hyperlipidemia, on statin therapy
  • BMI 29 kg/m²
Current status
  • Respiratory vaccinations: up to date
  • No hospitalizations in the past year
  • One moderate exacerbation 9 months ago, received a prescription but does not recall the details; one additional moderate exacerbation 1 month ago requiring OCS
  • Persistent exertional dyspnea and fatigue, takes days off here and there to recover, does not always seek medical attention
  • No daily sputum production, but reports “chest tightness” during viral illnesses
Investigations
  • CAAT score: 14
  • mMRC: 2
  • Post-bronchodilator FEV₁: 58% predicted
  • Blood eosinophils (single measurement): 360 cells/µL
  • Total IgE: normal
  • Alpha-1 antitrypsin level within normal range
  • Renal function: normal
  • CBC otherwise unremarkable
  • CT chest (performed 2 years ago): emphysema present; no new imaging available
